Inspect & adapt workshop
Inspect & Adapt is the SAFe event at the end of each Program Increment in which the ART reviews quantitative results (PI Objectives achieved, predictability measure, business value scoring), demos the integrated solution, and runs a structured problem-solving workshop on the top systemic improvements to commit to in the next PI.
I&A is SAFe's continuous-improvement mechanism at the program level — distinct from the team-level retrospective. The structure has three parts: the PI System Demo (the integrated solution running, not slide decks), the quantitative measurement review (predictability % per team, BV achieved, defect trends), and the problem-solving workshop (root-cause analysis on 2-3 prioritised systemic issues, with concrete improvement stories added to the next PI's backlog). The workshop discipline is the hardest part to get right: teams often surface symptoms (slow CI, blocked dependencies) without the root-cause discipline that produces durable improvement.
